What Is Hospice Care?

Considered to be the model for quality and compassionate care during the final stages of life, hospice care involves a team-oriented approach of palliative (comfort care) and support services designed to meet the unique needs of patients and families. Services are provided through a medically directed interdisciplinary team of professionals. Bereavement services also are available.

How Does Hospice Care Work?

Members of the hospice staff make regular intermittent visits to assess the patient and provide feedback to the physician on changes in the patient's condition. The Hospice staff also assesses the particular situation for additional care or services that are necessary for the patient's overall comfort and safety, as well as provide patients and families with community referrals for additional care and services. The hospice team, along with the patient and family, develop a plan of care focuses on the well being of the patient and family.

Our Hospice Team consists of an Attending Physician, a Medical Director, Nurses, Social Workers, Home Health Aides, Chaplains, Music Therapists, Massage Therapists and Volunteers. With the help of our dedicated team, you and your loved one can enjoy every day and live with dignity and respect.

We are accredited by The Joint Commission and are a certified Medicare and Medi-Cal provider. We also accept most commercial insurance plans.
